Choledochoscopic Electrohydraulic Lithotripsy for Biliary Stones

Abstract
Choledochoscopic lithotripsy with the aid of electrohydraulic lithotripter (EHL) was performed in 36 patients, including 12 patients with CBD stones, 20 patients with IHD stones, and 4 patients with GB stones. For the approach of EHL probe to
stones,
mother babyscope was used in 15 patients (9 patients with CBD stones and 6 patients with IHD stones), postoperative cholanigoscopy in 12 patients (2 CBD stones and 10 IHD stones), percutaneous transhepatic cholangioscopy in 5 patients (1 CBD and
4
IHD),
and percutaneous transhepatic cholecystoscopy in 4 patients with GB stones. Complete removal of the stones was achieved in 28 of 36 patients (78%). In one patient with CBD stones and seven patients with IHD stones, stones remained unremovable
because
the stones were very large or located deep in inaccessible branches of the IHD. There were no serious complications. Minor complications occured. including mild abdominal pain in 13 patients, and postprocedure ahills and fever in 3 patienrs.
These
results indicate that choledochoscopic lithotrtipsy with EHL is a safe and effective therapy in pateints with unextractable biliary stones by conventional endoscopic lithotripsies, such as basket or mechamcal lithotripsy.
